Sclerostin levels are elevated in individuals with Type 2 Diabetes and insulin resistance, and blocking sclerostin signaling improves insulin sensitivity in animal models.

High levels of sclerostin are linked to insulin resistance. While drugs targeting sclerostin (like romosozumab) exist for osteoporosis, their metabolic benefits are still being studied. For now, exercise and weight management remain the best ways to improve insulin sensitivity.

In a prospective cohort study (n = 1778), sclerostin levels were associated with fasting insulin levels and HOMA-IR... mice deficient in sclerostin (Sost-/-) have higher bone mass and lower visceral and subcutaneous fat mass, as well as improved insulin sensitivity

Why this rating

Strong association in humans and clear mechanistic benefit in knockout mice, but clinical translation is not yet established.
