Metabolic health status (MHO vs. MUO) is a better predictor of long-term cardiovascular risk and mortality than BMI alone, with metabolically unhealthy obesity (MUO) requiring prompt intervention.
If you have obesity, ask your doctor to check your metabolic health (blood pressure, lipids, blood sugar), not just your weight. Your risk of heart disease depends more on these numbers and where you store fat (belly vs. hips) than your BMI alone. If you are metabolically unhealthy, you need prompt intervention.
GoodQualifiesHIGH confidence
Epidemiological data report that MHO and MHNW have similar risks of CVD and mortality, meaning metabolic health rather than BMI may be a better surrogate of long-term risk... fat distribution rather than BMI is more strongly associated with cardiometabolic health
Why this rating
Supported by multiple epidemiological studies and the HUNT study cited in the review.

- Obesity is a chronic disease requiring comprehensive clinical evaluation and staging (e.g., EOSS, Kings criteria) rather than reliance on BMI alone, as BMI has low sensitivity for adiposity and fails to capture complication risk.Good
- Weight loss of 2.5-15% improves glycemic control in Type 2 Diabetes, while 5-10% weight loss may prevent knee pain in obesity-related osteoarthritis.Good
- GLP-1 receptor agonists (e.g., semaglutide, tirzepatide) and SGLT-2 inhibitors are preferred medications for obesity management in patients with cardiovascular or metabolic risk, as they promote weight loss and improve cardiovascular health.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
