Intensive glucose control provides minor cardiovascular benefits compared to lipid-lowering and anti-hypertensive therapies, and targeting very low glycemia may increase mortality risk in certain subgroups.
Do not rely solely on lowering your blood sugar to protect your heart. Managing your blood pressure and cholesterol levels, along with quitting smoking, offers significantly greater protection against cardiovascular disease than intensive glucose control alone. Discuss individualized HbA1c targets with your doctor, as overly aggressive lowering may be harmful for some.
We know now that benefits of such policy are minor in terms of vascular benefit, especially when compared with the much larger benefits stemming from lipid-lowering and anti-hypertensive therapy. In addition, aiming for very low glycemia target in some subgroups... may even be associated with an increase in mortality risk.
Why this rating
Based on multiple large-scale randomized controlled trials (Look AHEAD, ORIGIN, SAVOR, ADVANCE) cited in the review.

- Intensive lifestyle interventions (weight loss) do not significantly reduce cardiovascular event rates in Type 2 Diabetes patients who already have well-controlled baseline cardiovascular risk factors.Good
- Major calorie restriction via low-energy liquid diets can reverse Type 2 Diabetes by reducing ectopic fat in the liver and pancreas, restoring beta-cell function.Moderate

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
