HOPE-4 reported that this comprehensive model of care led by NPHW improved blood pressure control (69% in the intervention group vs 30% in the control group; p < 0.0001) and CVD risk (a reduction in the Framingham Risk Score for 10 year CVD risk was –11.2% in the intervention vs –6.4% in the control, P < 0.0001).
