Acarbose, an alpha-glucosidase inhibitor, reduces the risk of developing type 2 diabetes in patients with impaired glucose tolerance, but its effectiveness is lower than that of lifestyle modification and metformin.
Acarbose can help prevent diabetes in people with prediabetes by slowing down carb absorption, but it is less effective than lifestyle changes or metformin. It often causes bloating and stomach discomfort, leading many people to stop taking it. It is generally not the first choice for prevention.

Why this rating
Based on STOP-NIDDM and ACE trials cited in the review.

- Metformin therapy reduces the risk of developing type 2 diabetes in patients with prediabetes, with the greatest preventive effect observed in individuals with higher baseline glycated hemoglobin (HbA1c) and a history of gestational diabetes.Good
- Pioglitazone, a thiazolidinedione, significantly reduces the risk of developing type 2 diabetes in patients with impaired glucose tolerance and stabilizes beta-cell function, with a favorable cardiovascular safety profile compared to rosiglitazone.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
