Achieving recommended target values for glycated hemoglobin (HbA1c <7%), systolic blood pressure (<140 mmHg), and LDL-cholesterol (<100 mg/dl) in patients with type 2 diabetes is not associated with reduced all-cause mortality.
For patients with Type 2 Diabetes, strictly hitting the standard numerical targets for blood sugar, blood pressure, and cholesterol may not lower your risk of death as much as previously thought. While these targets are still used for other health reasons, do not assume that achieving them guarantees longevity. Focus on overall health stability and discuss individualized goals with your doctor, as aggressive treatment might not yield the expected mortality benefit.
In a real life clinical set of patients with type 2 diabetes mellitus, the combination of recommended target values of established cardiovascular risk factors is not associated with all-cause mortality.
Why this rating
Prospective observational cohorts with large sample sizes (n=1739) and long follow-up, though subject to confounding.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
