In patients with Type 1 Diabetes (T1D), weight loss interventions reduce insulin requirements but do not significantly improve glycemic control (HbA1c), unlike in Type 2 Diabetes where weight loss directly improves glycemic control.
If you have T1D and are losing weight, do not be discouraged if your HbA1c doesn't drop significantly. The main benefit is that you will likely need less insulin, which reduces your risk of hypoglycemia. Work with your provider to adjust your insulin doses as you lose weight to maintain safety, rather than expecting HbA1c to improve automatically.
However, there was no significant change in glycemic control between the two groups despite changes in weight and decreased insulin requirements... This is in contrast to meta-analyses of weight loss in T2D, in which glycemic control improved significantly with a total body weight loss of 5% or more... While improvement in glycemic control is often a desired outcome of weight loss in T2D, the relationship between excess weight and glycemic control is of much less clinical significance in the setting of T1D.
