Once-daily oral semaglutide 50 mg significantly reduces ad libitum energy intake, body weight, and appetite while improving control of eating in adults with obesity, without delaying gastric emptying at steady state.
Take oral semaglutide 50 mg once daily, following the specific dose-escalation schedule (starting at 1.5 mg and increasing every 4 weeks). Do not eat or drink (except water) for 6 hours before taking the tablet, and wait 30-45 minutes after taking it before your first meal. This regimen significantly reduces your energy intake and body weight primarily by lowering appetite and improving control over eating, rather than by slowing down your digestion.
In participants with obesity, once-daily oral semaglutide 50 mg reduced energy intake, body weight and appetite, and improved control of eating. There was no evidence of delayed gastric emptying at week 20, as measured through paracetamol absorption.
Why this rating
Randomized, double-blind, placebo-controlled trial with clear statistical significance, though limited to 20 weeks and single center.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
