Higher circulating levels of trans-palmitoleic acid (trans-16:1n-7), an exogenous fatty acid derived primarily from dairy fat, are associated with a significantly lower incidence of type 2 diabetes and improved metabolic risk factors, including lower insulin resistance and triglycerides.
Higher levels of trans-palmitoleic acid, a fatty acid found in dairy products, are linked to a lower risk of developing type 2 diabetes and better metabolic health. While this is an observational finding, it suggests that including whole-fat dairy as part of a balanced diet may be associated with these benefits, distinct from industrial trans fats.
Trans-palmitoleate was also associated with a substantially lower incidence of diabetes, with multivariate hazard ratios of 0.41 (95% CI, 0.27 to 0.64) and 0.38 (CI, 0.24 to 0.62) in quintiles 4 and 5 versus quintile 1 (P for trend < 0.001).
Why this rating
Large prospective cohort study with multivariate adjustments and validation in a second cohort, though residual confounding is acknowledged.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
