Initiating time-restricted eating (≤10h daily window) and high-intensity interval training (HIT) during the preconception period improves maternal insulin sensitivity and glycaemic control, thereby reducing the risk of gestational diabetes and adverse offspring metabolic outcomes.
Start optimizing your metabolic health before you conceive. Adopt a daily eating window of 10 hours or less to improve insulin sensitivity. Incorporate 2-3 short sessions of high-intensity interval training per week (e.g., 4-10 bursts of effort separated by rest). If pregnant, shorten the work bouts to under 60 seconds to ensure fetal safety. Focus on improving insulin sensitivity rather than rapid weight loss.
we propose that focusing on the timing of food may be a realistic option to improve glycaemic control for women with obesity/overweight or IGT who are planning a pregnancy... we advocate high-intensity exercise training as an enjoyable and time-efficient intervention to be commenced prior to and continued during pregnancy.
Why this rating
The paper is an opinion/review citing observational data and small-scale trials; large-scale RCTs in pregnant women are noted as lacking.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
