Tirzepatide, a dual GIP/GLP-1 receptor agonist, induces substantial weight loss and improves histological markers of metabolic dysfunction-associated steatohepatitis (MASH) and fibrosis in patients with type 2 diabetes and obesity.
If you have MASH and obesity, tirzepatide is a once-weekly injection that helps you lose weight and improves liver health markers. It starts at a low dose and increases slowly to minimize side effects. You should also follow diet and exercise advice provided by your care team.
Tirzepatide, a first-in-class once weekly glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor agonist, provided substantial weight reduction in patients with type 2 diabetes mellitus (T2DM) in the SURPASS studies... A post hoc analysis of patients from the phase 2 study in patients with T2DM showed that observed weight loss with tirzepatide was associated with improvements in biomarkers of MASH and fibrosis.
Why this rating
Based on a Phase 2b randomized controlled trial design with placebo control, though results are not yet fully reported in this protocol paper.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
