Tirzepatide produces significantly greater weight loss and glycemic improvement than semaglutide when used at comparable maintenance dosages and adherence levels.
If you are choosing between Semaglutide and Tirzepatide, Tirzepatide tends to produce greater weight loss (approx. 12% vs 8% average in this study). However, this benefit is only realized if you stay on the medication. If you struggle with side effects or cost, switching drugs may not solve the problem if you end up discontinuing.
In terms of the index medication, mean (SD) weight reduction at 1 year was 7.7% (9.0%; 95% CI: 7.4%–7.9%) with semaglutide and 12.4% (10.5%; 95% CI: 11.9%–12.9%) with tirzepatide.
Why this rating
Large observational cohort; consistent with RCT findings but subject to confounding.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
