Efpeglenatide, a long-acting GLP-1 receptor agonist administered once weekly or every two weeks, significantly reduces body weight and improves glycemic control (reverting pre-diabetes to normoglycemia) in patients with obesity or overweight without type 2 diabetes, regardless of baseline BMI, age, or pre-diabetes status.
If you have obesity or are overweight with health issues but no diabetes, efpeglenatide is a once-weekly (or bi-weekly) injection that can help you lose weight and improve your blood sugar control. It works regardless of your age or how much you weigh. You should also try to eat about 500 fewer calories a day and be more physically active. Common side effects are stomach-related, like nausea, but these often get better over time.
These results are consistent with the overall BALANCE population and suggest beneficial effects of efpeglenatide on glycemic control and body weight regardless of pre- diabetes status, age, or BMI at baseline.
Why this rating
The study is a randomized controlled trial (Phase II), but the subgroup analyses are exploratory with nominal p-values and no multiplicity adjustment, limiting statistical power.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
