Preoperative factors such as duration of diabetes, high HbA1c, and insulin use predict failure of T2DM remission after bariatric surgery.
If you have had diabetes for a long time or require high doses of insulin, your chances of complete remission after surgery are lower. This highlights the importance of considering surgery earlier in the disease course, before beta-cell function is severely compromised.
ModerateQualifiesMEDIUM confidence
Univariate analysis showed that duration of diabetes, higher preoperative HbA1c levels, and preoperative use of multiple medications were significant in predicting nonresponse after surgery. Multivariate analysis revealed that age, preoperative dose of insulin, and preoperative oral antidiabetics showed positive correlation to nonresponse.
Why this rating
Observational study with 82 patients; predictors are consistent with other literature but sample size is modest.

- Roux-en-Y gastric bypass (RYGB) induces remission of type 2 diabetes (T2DM) in patients with BMI 30-35 kg/m2, independent of significant weight loss, by altering gastrointestinal hormone secretion (GLP-1, GIP, GLP-2).Good
- Bariatric surgery prevents the onset of type 2 diabetes in obese patients without preexisting diabetes, with incidence rates significantly lower than non-surgical obese controls.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
