In patients with metabolic dysfunction-associated steatohepatitis (MASH) and fibrosis, achieving MASH resolution and fibrosis improvement is significantly associated with greater reductions in body weight, improved glycemic control (HbA1c), and normalization of liver fat.
If you have MASH with fibrosis, achieving significant weight loss, better blood sugar control, and normalizing liver fat are key steps toward resolving the disease. Tirzepatide, taken once weekly and titrated up from 2.5mg to 5-15mg over several months, has been shown to facilitate these metabolic changes, which in turn drive histological improvement in the liver.
In the overall study population, responders for both MASH resolution and fibrosis improvement had greater reductions in weight, HbA1c, and normalization of liver fat compared with nonresponders.
Why this rating
Post hoc analysis of a phase 2 randomized controlled trial (SYNERGY-NASH) with a substantial sample size (n=154 analyzed), though exploratory.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
