Females with severe obesity face a disproportionately higher relative risk for stroke, total CVD, and all-cause mortality compared to males with similar obesity levels.
Women with obesity should be particularly vigilant about stroke risk, as their relative risk increases more sharply with obesity severity than men's. Regular cardiovascular screening is essential for women with higher BMI.
StrongQualifiesHIGH confidence
Compared to males, females experienced higher relative increases in the risk associated with severe obesity for outcomes stroke, CVD, and all-cause mortality (p-for-interaction: <0.001, 0.003, and <0.001, respectively).
Why this rating
Large sample size allows for robust subgroup analysis by sex.

- Higher BMI, particularly severe obesity (Class 2 and 3), is robustly associated with increased risk of heart failure and atrial fibrillation, independent of traditional risk factors.Strong
- Overweight status (BMI 25-<30) is not associated with increased risk for most cardiovascular outcomes and may have a neutral or slightly protective effect on all-cause mortality compared to normal weight.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong

- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
