Central (aortic) blood pressure is a stronger predictor of future cardiovascular risk and target organ damage than peripheral (brachial) blood pressure because vital organs are more directly exposed to central pressure.
If you have obesity or hypertension, ask your doctor about central blood pressure assessment. Standard arm-cuff readings may underestimate the stress on your heart and kidneys. Central pressure monitoring offers a more accurate prediction of long-term cardiovascular risk and organ damage.
GoodSupportsHIGH confidence
recent evidence suggests that aortic (central) BP has a greater association with future cardiovascular events than peripheral BP. This is because the heart, kidneys, and cerebral vasculature are more exposed to aortic pressure than to brachial pressure.
Why this rating
Supported by multiple cited studies (Strong Heart Study, etc.) and physiological reasoning, though the paper itself is a review.

- Bariatric surgery significantly reduces peripheral blood pressure and leads to hypertension remission in a majority of patients with morbid obesity, primarily through weight loss.Strong
- There is currently insufficient evidence to determine the specific impact of obesity on central blood pressure and its modification after bariatric surgery, as no studies have evaluated 24-hour central BP changes at medium or long term post-surgery.Strong
