Tirzepatide, a dual GIP and GLP-1 receptor agonist, provides superior weight loss and metabolic improvements compared to selective GLP-1RAs, insulin, and placebo.
Tirzepatide is a once-weekly injection that targets two hormones (GIP and GLP-1) to improve blood sugar and promote weight loss. Clinical trials show weight loss of 15-21% depending on the dose. It is approved for Type 2 Diabetes and is being studied for obesity. It is more effective than some other GLP-1 drugs and insulin in terms of weight loss and metabolic markers.
In clinical trials enrolling patients with T2DM and also obese patients without T2DM, when compared to selective GLP-1RAs, tirzepatide showed superiority in the key aspects: weight loss, improvement in LDL and triglycerides, and improvement in cardiovascular risk, glucose control, and HbA1c reduction
Why this rating
Based on FDA approval from nine clinical trials involving over 7,000 subjects.

More from this paper
- GLP-1 receptor agonists (GLP-1RAs) such as semaglutide and liraglutide facilitate weight loss and glycemic control by delaying gastric emptying, stimulating insulin release, and suppressing glucagon secretion.Good
- Semaglutide demonstrates superior weight loss and glycemic control compared to other GLP-1RAs like liraglutide and dulaglutide, with weight loss ranging from 10% to over 20%.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
