In patients with high triglycerides or metabolic syndrome, non-HDL cholesterol is a superior predictor of cardiovascular risk and a valid secondary treatment target compared to LDL cholesterol alone.

Many studies have demonstrated that non-HDL cholesterol is a better predictor of CVD risk than is LDL cholesterol... and this may be especially true of statin-treated patients... ATP III introduced non-HDL cholesterol as a secondary treatment target, recognizing that in this patient population, LDL cholesterol underestimates the burden of atherogenic, cholesterol-carrying lipoproteins.
