Roux-en-Y gastric bypass (RY-GBP) surgery significantly enhances postprandial secretion of incretins (GLP-1 and GIP) and restores the incretin effect on insulin secretion within one month, independent of significant weight loss.
For patients with type 2 diabetes undergoing Rou-en-Y gastric bypass, the surgery triggers a rapid increase in gut hormones (GLP-1 and GIP) that significantly improves insulin secretion within one month. This hormonal shift is a key driver of early diabetes remission, occurring before significant weight loss is achieved. This mechanism supports the development of non-surgical therapies that target these same incretin pathways.
One month after RY-GBP, body weight decreased by 9.2 ± 7.0 kg, oral glucose-stimulated GLP-1 (AUC) and GIP peak levels increased significantly... The blunted incretin effect markedly increased from 7.6 ± 28.7 to 42.5 ± 11.3 (P < 0.005) after RY-GBP... These data suggest that early after RY-GBP, greater GLP-1 and GIP release could be a potential mediator of improved insulin secretion.
