In men with type 2 or type 1 diabetes, lower testosterone levels (both total and calculated free testosterone) are independently associated with higher insulin resistance, and longitudinal changes in testosterone inversely correlate with changes in insulin sensitivity.
If you have diabetes and low testosterone, it is likely linked to your insulin resistance rather than just aging. Improving insulin sensitivity through weight loss and exercise may help normalize testosterone levels, though testosterone replacement therapy is not yet proven to be the primary solution.
Low testosterone levels were independently associated with insulin resistance in men with type 1 diabetes as well as type 2 diabetes. Serial measurements also revealed an inverse relationship between changes in testosterone levels and insulin resistance.
Why this rating
Large cohort (n=649), rigorous statistical adjustment for confounders (age, BMI), and longitudinal follow-up in a subgroup.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
