Lifestyle interventions (diet, exercise, or behavioral) significantly improve anthropometric, metabolic, and reproductive markers in women with PCOS, specifically reducing body weight, waist circumference, fasting insulin, and testosterone levels.

For women with PCOS, engaging in structured lifestyle changes—whether through diet, exercise, or behavioral counseling—can significantly improve key health markers. Specifically, these interventions can lead to modest but meaningful reductions in body weight, waist circumference, and insulin levels, as well as improvements in testosterone levels and hirsutism. While lifestyle changes did not show significant effects on glucose tolerance or lipid profiles in this review, they remain a first-line recommendation for managing the underlying hormonal and metabolic disturbances of PCOS.

For secondary outcomes, lifestyle intervention provided benefits when compared to minimal treatment for endpoint values for total testosterone (mean difference (MD) -0.27 nmol/L...), hirsutism by the Ferriman-Gallwey score (MD -1.19...), weight (MD -3.47 kg...), waist circumference (MD -1.95 cm...), waist to hip ratio (MD -0.04...), fasting insulin (MD -2.02 µU/mL...) and oral glucose tolerance test insulin... and per cent weight change (MD -7.00%)
