In moderately obese men, elevated circulating free fatty acid (FFA) levels drive the accumulation of triglycerides within the myocardium and epicardial fat, a process that precedes left ventricular hypertrophy and overload.

For individuals with obesity, high levels of free fatty acids in the blood can lead to fat storage inside the heart muscle, even before heart function declines. This suggests that managing overall metabolic health and fatty acid availability is crucial for preventing early cardiac structural changes.
