PPAR-gamma agonists (thiazolidinediones) reduce liver fat and increase adiponectin levels, whereas metformin does not significantly reduce liver fat content.

If you have fatty liver, standard diabetes medication like metformin may not reduce the liver fat itself, whereas PPAR-gamma agonists (like pioglitazone or rosiglitazone) can significantly reduce it, similar to the effect of moderate weight loss. Consult a doctor for medication options.

In addition, treatment of both lipodystrophic and type 2 diabetic patients with peroxisome proliferators activator receptor-gamma (PPAR-gamma) agonists, but not metformin, decreases liver fat and markedly increases adiponectin levels.
