Continuous Positive Airway Pressure (CPAP) treatment for OSA does not consistently improve glucose metabolism (HbA1c or insulin sensitivity) in randomized controlled trials, particularly when adherence is low or treatment duration is short.

CPAP is not a magic bullet for blood sugar control. Its benefits for glucose metabolism are inconsistent and often depend on how many hours you actually use it each night. If you are prescribed CPAP, focus on maximizing your nightly usage time, as higher adherence is linked to better metabolic outcomes.

Overall, only three of the nine studies (Lam et al., 2010; Sharma et al., 2011; Weinstock et al., 2012) reported positive findings during the randomized treatment period... Nightly treatment use (CPAP or sham-CPAP) was 5 h or less in most studies... In this study, the average nightly CPAP use was only about 3.3 h and no correlation was reported between CPAP compliance and measures of insulin resistance and A1c.
