In Chinese adults with type 2 diabetes, higher levels of abdominal obesity indices (waist circumference, visceral adiposity index, lipid accumulation product, and Chinese visceral adiposity index) are significantly associated with an increased risk of incident cardiovascular events, with CVAI demonstrating the strongest predictive performance.
For people with type 2 diabetes, monitoring waist circumference or using calculated indices like CVAI is more critical for predicting heart disease risk than BMI alone. Reducing abdominal fat deposition is a key intervention strategy, particularly for those who are not overweight by BMI standards but carry visceral fat.
After multivariable adjustment, each 1-SD increase in WC, VAI, LAP, and CVAI was associated with a higher risk of CV events in people with T2D, with adjusted hazard ratios (HRs) being 1.57 [95% CI (confidence interval): 1.39–1.78], 1.11 (95% CI 1.06–1.16), 1.46 (95% CI 1.36–1.57), and 1.78 (95% CI 1.57–2.01), respectively.
Why this rating
Prospective cohort study with large sample size (n=2328) and long follow-up (median 59 months), though observational design limits causal inference.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
