Tirzepatide (TZP) administration in adults with overweight or obesity significantly reduces total fat mass and visceral adipose tissue compared to baseline, placebo, and other anti-obesity medications like dulaglutide and semaglutide.
Tirzepatide is a highly effective treatment for reducing body fat and visceral fat in people with overweight or obesity, outperforming other common medications like semaglutide and dulaglutide in fat reduction metrics. However, users should be aware that significant fat-free mass (muscle) loss also occurs, though the net benefit of fat reduction remains superior. Treatment involves weekly injections, typically titrated up to 15mg over several months.
TZP was shown to result in a significant reduction in total fat mass (FM), visceral adipose tissue (VAT) and waist circumference (WC) between baseline and short as well as intermediate follow-ups. Compared to other anti-obesity medications (e.g., dulaglutide and semaglutide) taken over the same duration, TZP showed a superior decrease in body fat compartments (i.e., total FM, VAT and WC).
Why this rating
Based on a systematic review of 6 high-quality RCTs with low risk of bias.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
