Orforglipron, an oral non-peptide GLP-1 receptor agonist, demonstrates efficacy comparable to injectable GLP-1 RAs with fewer dosing restrictions, achieving up to 2.10% HbA1c reduction and 9.6% weight loss.
Orforglipron is an oral, non-peptide GLP-1 receptor agonist that offers a convenient once-daily dosing without the strict food and water restrictions of other oral GLP-1 RAs. In Phase 2 trials, it achieved significant HbA1c reduction (up to 2.10%) and weight loss (9.6%), comparable to injectable options. It is currently in Phase 3 trials.
After 26 weeks of treatment, the mean change in HbA1c with orforglipron ranged from −1.19% to −2.10% (maximum effect with orgorglipron 45 mg)... Moreover, mean body weight reduced by 9.6% with orforglipron 45 mg
Why this rating
Based on Phase 2 trial data, with Phase 3 trials ongoing.

- Tirzepatide, a dual GLP-1/GIP receptor agonist, achieves superior HbA1c reduction (up to 2.6%) and weight loss (up to 15.7%) compared to other GLP-1 RAs and insulin, making it a highly effective treatment for T2D and obesity.Strong
- Oral semaglutide at 50 mg once daily achieves significantly greater HbA1c reduction (2.1%) and weight loss (9.8%) compared to the approved 14 mg dose, with higher discontinuation rates due to gastrointestinal adverse events.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
