Weekly subcutaneous administration of Tirzepatide (5-15 mg) significantly reduces body weight and body fat percentage in non-diabetic obese patients compared to placebo, with effects comparable to bariatric surgery.
If you are obese and non-diabetic, Tirzepatide is a highly effective weekly injection that can lead to significant weight loss (up to ~21%) and fat loss, outperforming previous GLP-1 drugs. It requires a commitment to weekly injections and lifestyle changes (diet and 150 mins/week exercise), but may offer a better side-effect profile regarding nausea than older drugs due to its dual GIP/GLP-1 mechanism.
Tirzepatide significantly reduced the body weight and other obesity parameters, as compared to the placebo... The degree of weight-loss of obese patients in the SURMOUNT-1 trial could only be compared to the results of using bariatric surgery.
Why this rating
Based on a large randomized controlled trial (n=2539) with statistically significant results (p<0.001), though it is a single study analysis rather than a meta-analysis.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
