Post-bariatric surgery patients, particularly those undergoing Roux-en-Y gastric bypass (RYGB) or biliopancreatic diversion (BPD), are at high risk for postprandial hyperinsulinemic hypoglycemia (PHG) due to accelerated carbohydrate delivery to the small intestine, which triggers excessive GLP-1 secretion and subsequent insulin hypersecretion.
If you have had gastric bypass surgery, be aware that eating simple sugars can cause a dangerous drop in blood sugar 2-3 hours later. This is caused by your body overproducing insulin in response to fast-absorbing carbs. Manage this by strictly limiting simple sugars and focusing on complex carbohydrates and fiber to slow absorption.
Согласно последним исследованиям, ведущую роль в неадекватной избыточной выработке инсулина поджелудочной железой играет гиперсекреция инкретинов в ответ на ускоренное поступление углеводов в тонкую кишку.
Why this rating
Based on a comprehensive review of multiple studies, including GLP-1 antagonist trials (exendin) and continuous glucose monitoring data.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
