The hepatic lipase (HL) promoter polymorphism (HL-514T allele) modulates the effect of physical activity on HDL-C levels, with carriers of the T allele showing a blunted response to exercise-induced HDL increases.
If you have certain genetic variants (HL-514T), you might not see as big a boost in 'good' cholesterol (HDL) from exercise as others. This doesn't mean exercise is useless, but it suggests you might need a more targeted approach to managing heart disease risk.
ModerateQualifiesMEDIUM confidence
Individuals with the HL-514T allele are more resistant to exercise associated higher HDL-C.
Why this rating
Observational study of gene-environment interaction; requires validation in other populations.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
