Tirzepatide (5 mg, 10 mg, and 15 mg) significantly reduces body weight in patients with type 2 diabetes and obesity compared to placebo, GLP-1 receptor agonists, and basal insulin.
Tirzepatide is a highly effective treatment for weight loss in people with T2DM or obesity, outperforming other common medications like GLP-1 agonists and insulin. It is taken as a once-weekly injection. While it causes gastrointestinal side effects like nausea and diarrhea, these are usually mild and temporary. If you are considering this, discuss the benefits of significant weight loss against the potential for temporary stomach issues with your doctor.
In total, ten studies (12 reports) involving 9,873 patients were identified. A significant loss body weight in the tirzepatide group versus the placebo by -9.81 kg (95% CI (-12.09, -7.52), GLP-1 RAs by -1.05 kg (95% CI (-1.48, -0.63), and insulin by -1.93 kg (95% CI (-2.81, -1.05), respectively.
Why this rating
Based on a meta-analysis of 10 RCTs involving nearly 10,000 patients, though heterogeneity was high in some comparisons.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
