Combining GLP-1 receptor agonists (Liraglutide or Exenatide) with Metformin significantly improves pregnancy rates and endometrial receptivity compared to Metformin monotherapy in women with PCOS undergoing IVF.
If you have PCOS, are overweight, and are struggling with infertility despite trying standard treatments like Clomiphene, ask your doctor about adding Liraglutide to your Metformin regimen before starting IVF. One study showed this combination significantly improved pregnancy rates per embryo transfer compared to Metformin alone. Note that these drugs are not approved for use during pregnancy, so a washout period is required.
After 1 year of follow-up, pregnancy was achieved in 69.2% of women in the combination arm and in 35.4% of women in the monotherapy arm. The PR per ET was significantly higher (85.7%) in the participants in the combination arm compared to 28.6% in the other group (p = 0.03).

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
