Intentional weight loss of 7–14% of initial body weight typically enables the discontinuation of anti-diabetic medications in overweight or obese patients with type 2 diabetes.
If you have type 2 diabetes and are overweight, losing 7-14% of your body weight through a structured program (diet, exercise, behavior change) can significantly reduce or eliminate your need for diabetes medications. This should be done under a doctor's supervision, as your medication doses will likely need to be adjusted as you lose weight.
Among overweight or obese patients with type 2 diabetes, intentional weight loss of 7–14% was typically required for full discontinuation of at least one anti-diabetic medication.
Why this rating

- For every 5% weight loss, the predicted dose reduction for sulfonylureas, insulin, and any anti-diabetic medication is 39%, 42%, and 49%, respectively.Moderate
- Discontinuation of insulin is achieved at a mean weight reduction of 11% of initial body weight, while sulfonylureas require 14% and sitagliptin 7.1%.Moderate

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
