Aggressive blood glucose lowering (targeting HbA1c < 6.5%) in type 2 diabetes does not necessarily reduce cardiovascular risk and may increase overall mortality due to side effects like hypoglycemia and weight gain.
Do not aim for an HbA1c below 6.5% if it requires strong medications that cause low blood sugar or weight gain. The goal is stable, safe blood sugar levels, not the lowest possible number.

- Multifactorial intervention targeting blood glucose, blood pressure, and lipids reduces cardiovascular mortality in type 2 diabetes more effectively than standard care focused only on glucose.Strong
- Intensive lifestyle interventions (diet and exercise) aiming for weight loss and increased physical activity do not significantly reduce cardiovascular morbidity or mortality in patients with type 2 diabetes compared to standard care, despite improving surrogate markers.Good

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
