Tirzepatide achieves body weight loss of ≥ 5% significantly faster than semaglutide 1 mg in patients with type 2 diabetes.
If your goal is weight loss alongside blood sugar control, tirzepatide (10 or 15 mg) will likely get you to a 5% body weight reduction in about 12 weeks, which is half the time it takes with semaglutide 1 mg. This benefit persists despite the slower dose escalation schedule required to manage side effects.
In SURPASS-2, the median time to first achieve a body weight loss of > 5% was 16.0 weeks (5 mg) and 12.4 weeks (10 and 15 mg) with tirzepatide than with semaglutide 1 mg (24.0 weeks).
Why this rating
Based on randomized controlled trial data (SURPASS-2) with clear time-to-event analysis.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
