Lean body mass is preserved on average in both TRF and MBD groups, but individual responses vary significantly, with some participants losing lean mass despite adequate protein intake.
While your group average lean mass stays stable, individual results vary. Ensure you are lifting weights and eating enough protein (1.4-2.0g/kg body weight or 2.3-3.1g/kg lean mass) to maximize the chance of preserving muscle while losing fat.

For LBM, however, no significant change could be observed in either group... Nevertheless, when looking at the individual changes, it was noticed that some participants lost significant LBM and others gained LBM.
Why this rating
RCT data shows group-level preservation but individual variability.

- Bariatric surgery (VSG and RYGB) is the most effective and sustainable treatment for obesity, producing marked and sustained weight loss that non-surgical interventions cannot match.Strong
- Severely protein-deficient diets (2–3% energy) induce lean body mass loss and metabolic imbalance, whereas adequate protein intake (0.66 g/kg/d minimum) is a prerequisite for maintaining muscle, bone, and physiological function.Strong
- For patients with Class III obesity (BMI >39.9 kg/m2), metabolic bariatric surgery (MBS) is significantly more effective than any currently approved medication, with BilioPancreatic Diversion (BPD) showing the highest estimated weight loss.Strong
