Combining HMB (3g/day) with Resistance Training does not provide a statistically significant additional benefit to muscle mass or strength compared to Resistance Training alone in elderly sarcopenic patients after hip surgery.
You do not need to take HMB if you are doing Resistance Training after hip surgery. RT alone is enough to build muscle and strength; adding HMB does not provide extra benefit for these metrics.

Comparison among the four groups showed that there were no differences in the change of WBFFM, UEM, HLESMM, ILESMM, BSMM, and SMI between the HMB + RT group and the RT group postoperatively at 3 months (P > 0.05)

- Resistance training (RT) significantly improves body composition (muscle mass) and grip strength in elderly patients with sarcopenia following hip replacement surgery, regardless of HMB supplementation.Moderate
- HMB supplementation alone (3g/day) prevents the decline in muscle mass and grip strength in elderly sarcopenic patients after hip surgery, but does not significantly increase muscle mass or strength compared to baseline.Moderate
