Combining tirzepatide with low-energy ketogenic therapy (LEKT) produces significantly greater reductions in hepatic steatosis and liver stiffness than combining tirzepatide with a low-calorie diet (LCD), independent of total body weight loss.
If you have MASLD and are using tirzepatide, adding a structured low-energy ketogenic diet (under 30g carbs, ~1200 kcal) for 12 weeks may reduce liver fat and stiffness more effectively than a standard low-calorie diet, even if your total weight loss is similar. This requires strict adherence and monitoring, ideally under medical supervision, to ensure safety and efficacy.
Both groups showed significant reductions in body weight... with no significant intergroup difference... CAP and LSM improved significantly in both groups, but reductions were greater in the TZP + LEKT group (CAP −12.5%, p < 0.001; LSM −22.7%, p < 0.001) versus LCD (CAP −6.7%, p = 0.014; LSM −9.2%, p = 0.022).
