Semaglutide 2.4 mg administered once weekly for 68 weeks significantly improves health-related quality of life (HRQoL) utility scores compared to placebo in adults with overweight or obesity.
If you are an adult with overweight or obesity, taking 2.4 mg of semaglutide once weekly for about 16 months, alongside diet and exercise changes, is likely to significantly improve your daily health quality of life compared to placebo. This benefit is particularly pronounced if you have a higher BMI (over 40) or specific conditions like knee osteoarthritis or sleep apnea.
The estimated mean treatment difference in SF-6D utility score favoured semaglutide 2.4 mg, and, at week 68, it was 0.057 (0.038-0.076) for the FAS.
Why this rating
Derived from a large, randomized, double-blind, placebo-controlled trial (STEP 1) with robust statistical modeling (MMRM).

- Each unit increase in baseline BMI is associated with a statistically significant decrease in health utility scores, indicating that higher obesity severity correlates with lower quality of life.Good
- Higher baseline BMI and the presence of symptomatic comorbidities (specifically knee osteoarthritis or obstructive sleep apnoea) are associated with greater improvements in health utility scores following semaglutide 2.4 mg treatment.Moderate

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
