Vitamin D3 supplementation alone is the most cost-effective management strategy for sarcopenia in Iran, whereas combination therapies (Protein+Vitamin D, Protein+Vitamin D+Exercise) are not cost-effective despite higher clinical effectiveness.

For healthcare systems or individuals in Iran, Vitamin D3 supplementation is the most economically sensible intervention for managing sarcopenia. While adding protein and exercise improves health outcomes (QALYs), the high cost of these combinations makes them not cost-effective compared to Vitamin D alone. Focus on Vitamin D for the best economic return.
