For severe obesity, laparoscopic surgery (like gastric bypass or sleeve) currently offers the highest chance of significant, sustained weight loss (20-30%) and diabetes remission compared to drugs or diet alone. However, it requires lifelong vitamin supplementation and carries surgical risks. New medications (GLP-1s) are closing the gap, offering 15-20% loss, but surgery remains the most effective option for those who qualify.
