Roux-en-Y gastric bypass (RYGB) surgery significantly reduces the incidence of new-onset type 2 diabetes and hypertension compared to non-surgical controls over a 6-year period.
RYGB surgery not only aids weight loss but also drastically lowers the risk of developing new-onset type 2 diabetes (89% reduction in odds) and significantly improves hypertension remission rates compared to non-surgical management in severely obese patients.
GoodSupportsHIGH confidence
The incidence of diabetes throughout the course of the study was reduced after RYGB surgery (2%; 95% CI, 0%-4%; vs 17%; 95% CI, 10%-24%; OR, 0.11... compared with control group 1)... Remission rates of hypertension at year 6 remained significantly improved in the RYGB surgical group compared with 2 control groups (42%; 95% CI, 32%-52%; vs 18%; 95% CI, 9%-27%)
Why this rating
Same study as N1, robust design.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
- For patients with specific monogenic obesity syndromes (leptin deficiency, POMC/PCSK1/LEPR mutations), targeted pharmacotherapy (recombinant leptin or setmelanotide) is highly effective and should be prioritized, unlike in polygenic obesity.Strong
- Continued weekly administration of 2.4 mg subcutaneous semaglutide prevents weight regain and promotes further weight loss in adults with overweight or obesity, whereas switching to placebo results in significant weight regain.Strong
