Biliopancreatic diversion (BPD) achieves sustained weight loss and maintenance primarily by establishing a fixed intestinal energy absorption threshold for fat and starch, rather than through caloric restriction or reduced appetite.
This surgical intervention works by physically preventing the absorption of calories from fats and starches, creating a hard ceiling on energy intake regardless of how much you eat. It is not a diet; it is a physiological limit. Patients often regain normal appetite and eat larger volumes, but the weight stabilizes because the excess calories are excreted rather than stored.
The indefinite weight maintenance appears to be due to the existence of a threshold absorption capacity for fat and starch, and thus energy... the final weight loss depends on the reduced energy intestinal absorption.
