The 'Obesity Paradox' (lower mortality in obese patients with certain chronic diseases) is largely an artifact of using BMI, which fails to distinguish between high fat mass and high muscle mass, and ignores the inflammatory status of visceral fat.
If you have a chronic condition, do not try to gain weight to 'beat' the odds. Focus on preserving muscle mass through resistance training and adequate protein. The 'paradox' is a measurement error; high muscle is good, high visceral fat is bad.
