Level 3 portable sleep tests demonstrate high diagnostic accuracy (AUC 0.85–0.99) for moderate to severe obstructive sleep apnea in adults with high pretest probability and no unstable comorbidities, yielding clinical management outcomes equivalent to Level 1 polysomnography.
If you suspect you have moderate to severe sleep apnea (loud snoring, witnessed breathing pauses, daytime sleepiness) and have stable health, a home sleep test is a valid, accurate, and convenient alternative to an overnight lab study. It provides the necessary data to start treatment (like CPAP) with outcomes equivalent to the more expensive and time-consuming lab test. However, if you have other complex sleep disorders or unstable heart/lung conditions, you still need the comprehensive lab test.
Level 3 portable devices showed good diagnostic performance compared with level 1 sleep tests in adult patients with a high pretest probability of moderate to severe obstructive sleep apnea and no unstable comorbidities.
Why this rating
Based on a systematic review and meta-analysis of 59 studies (5026 patients) with moderate to high heterogeneity but strong summary statistics.
