Variations in soft tissue hydration introduce systematic, predictable errors in Dual-Energy X-ray Absorptiometry (DXA) fat estimates, with the magnitude of error depending on the elemental composition of the excess fluid and the degree of overhydration.
If you are using DXA to track body fat changes, ensure your hydration status is stable between scans. Significant fluid shifts (like severe edema or dehydration) can skew results, making you appear leaner or fatter than you are. For healthy individuals with normal hydration, the error is likely negligible (<1%), but for those with fluid retention issues, DXA fat estimates may be inaccurate.
Results indicate that systematic errors in DXA percent fat arise with added fluids when fractional masses are varied as a percentage of combined fluid + soft tissue mass. Three independent determinants of error magnitude were established: elemental content of overhydration fluid, fraction of combined fluid + soft tissue as overhydration fluid, and initial soft tissue composition.
Why this rating
The study uses rigorous in vitro physical modeling and validation with known phantoms, providing high internal validity for the mechanism, though it lacks in vivo human confirmation of the magnitude of error in pathological states.
