Total activation of PPAR-gamma (e.g., via rosiglitazone) can be detrimental to liver histology by promoting hepatic lipogenesis, whereas partial activation (e.g., via telmisartan or pioglitazone) improves insulin sensitivity and reduces steatosis without the harmful lipid accumulation.

Not all diabetes medications that target PPAR-gamma work the same way for fatty liver. Total activation might increase liver fat, so partial activation strategies (like those with telmisartan or pioglitazone) are often preferred to improve insulin sensitivity without worsening liver fat.
