Energy balance
The magnitude of systolic blood pressure reduction with GLP-1 RAs is directly associated with changes in body weight, BMI, and HbA1c, suggesting these metabolic improvements drive the BP benefit.
The blood pressure lowering effect of GLP-1s like semaglutide is largely driven by the weight loss and metabolic improvements (lower A1c/BMI) they cause, rather than a direct effect on blood vessels alone.
GoodQualifiesMEDIUM confidence
Among semaglutide cohorts, mean change in hemoglobin A1c and mean change in body mass index were directly associated with SBP reduction.
Why this rating
Meta-regression of RCTs.

- GLP-1 receptor agonists (semaglutide, liraglutide, dulaglutide, exenatide) produce a statistically significant but clinically modest reduction in systolic blood pressure (SBP) compared to placebo, with effects ranging from -1.46 to -3.40 mmHg.Good
- GLP-1 receptor agonists generally do not significantly reduce diastolic blood pressure (DBP), with the exception of exenatide.Good
