Current clinical-metabolomic biomarkers (including BCAA and lipid profiles) cannot predict individual changes in insulin sensitivity following lifestyle interventions, despite their ability to correlate with baseline insulin levels.
Do not rely on current commercial metabolomic tests (like BCAA or lipid profiling) to predict whether you will lose weight or improve insulin sensitivity through exercise or diet. These tests may tell you your current risk level, but they cannot tell you if a specific intervention will work for you. Monitor actual physiological changes using high-sensitivity insulin assays if possible.

Thus, we demonstrate that the choice of insulin assay is critical when quantifying the influence of life-style on fasting insulin, while none of the clinical-metabolomic biomarkers, validated in cross-sectional data, are suitable for monitoring longitudinally changes in IR status.

- Initial treatment for type 2 diabetes should be a combination of metformin and either an SGLT-2 inhibitor or a GLP-1 receptor agonist to achieve cardiorenal protection, rather than monotherapy or older agents like sulfonylureas.Strong
